Personally, just like how we don't modify helmets for people with big ears, I think helmet design would remain as we see it now. Primarily due to safety as those tall ear points would lead to a torque on the neck that could severely injure the wearer in a crash. I know animal "ears" on helmets exist, but they can break away without causing injury (or they should). If the ones in the image were designed to break away, that would lead to injury to her ears in multiple ways.

In-universe it's a bit of a mixed bag. Just looking at official arts, we have post #9529572 which have Digital wearing a helmet with ears sticking out unprotected while post #10214548 and post #10319201 show helmets without the ears exposed.
