Looking at wikipedia the character Asato Ai is/was a man (the "girl" on top). Going by the myanimelist description of the character, the character is a transsexual. Going by wikipedia, it would sound like the character is a cross-dresser.

The image is also tagged Shemale and Newhalf (transsexual) on pixiv.

No it is not.
She is a newhalf (shemale/transsexual); they are not the same as futanari.
