I think the artist is using c2 long lens perspective which would minimise the foreshortening distortion (the way things appear larger as they come towards you). you would need to be looking from a long way away in order for that to work however, which is a bit strange considering it's a top down shot of them lying ona bed. if it's indoors, the room would have a very high ceiling.
