Suika is still strong enough to intercept without going in the ring. Since she's in the opposite of the ring I guess she enlarged her whole arm as opposed to just the palm to reach Okuu, so Okuu won't be able to enter the ring normally since the whole ring is practically within her reach.
Things are looking bad for Heavenly Sky, but still, hold on Okuu! Find your way out of this! And Tenshi, figure out a way to try and break free!(which I think might happen in the next strip, at least I hope) You girls can do it!

No, she went fully super sized. You can tell by judging perspective between the first panel (Suika is shorter than the top rope) and in the last panel (Suika is taller than the rope, Koakuma, etc.). Also, motion lines indicating she's getting smaller again. The effort appears to be a bit tiring.
Sakuya keeps switching her hold; asides from the aforementioned pain and temporarily restraining her, it doesn't seem like those are having any kind of longterm effect on Tenshi's fighting ability.
