It's a reference to a tongue-eating louse, a parasite that attaches itself to the mouth of a fish to steal a share of the fish's food. (And both a male and female go in as a pair, so they are basically constantly doing it in the stump where the tongue used to be...)

I'd sooner bite off my own tongue to kill the parasite and risk the aftermath than live with one of those inside me!
Now for a while I'm going to second guess everything I eat because I'm paranoid it might have this parasite hiding in it waiting to replace my tongue.

According to what I read, it doesn't attach to your tongue. It kills it off, and then replace it. That thing replaces your tongue. After your tongue fell off. Sweet dreams, mate~
