Basically the evil twin set her married sister up with a punk who blackmailed her with a video of the twin, meanwhile the twin took the married wife's position. Since they always did that play before they married the husband never noticed until the twin herself revealed it showing him a video of his real wife being fucked by the punk. All of this while riding him, impregnating herself of the husband in the process.
TL;DR Double netorare. Twin is yandere and didn't like that her sister got the guy she wanted. Got the husband and is pregnant of him, while the wife, her sister, is mindbroken while still banged by the punk.
