What kind of game is that, where you can actually lose your save files? I think something like that's only happened to me in one game I've ever played, and that was Metroid Fusion telling me my file was corrupted. I didn't even lose my file, it just reverted my progress to the previous time I'd saved.

Mostly with bugged saving/loading code. Some games also make several files for a single state and mixing wrong files can kill your save.
There was game even more cruel (can't recall it's name though): there's two trapped characters somewhere underwater. You have unlimited tries, when practicing with underwater probe. But while doing "rescue" run and fail, that character's data being physically deleted from game's folder.
