Well, it's KISS and they made Custom Maid 3D 2. While modders do a good job at revamping the graphics and whatnot, there's only so much you can do including environment stuff. Since it's also done in Unity, you can't modify the props and/or edit the scene to fix chain clipping, etc. KISS are known to cut corners and just push stuff to market for quick bucks.

I'm familiar with CM3D2, no complaints with it. Heck, a better angle to avoid the chain and male model being in-frame and better breast settings and it'd look pretty good.
I'm more talking about the fact that somebody looked at this and thought "Yes, this is Danbooru quality".
