That's most likely Father Glasses or an older nun, dragged by the duckies. Note the red rope held in the mouth of the duck and the same colored rope in his wrist. If you looked closely at the carrot, you can see 'today' written on it. Maybe Smug was abandoned?

I guess this gives us some insight into why it was Scowly who took responsibility to raise the Protagonist Nun. Herself being abandoned, she couldn't allow the baby to suffer same fate.

Given where it sits on her chin as an adult, her face is probably just contorted such that it's "off-camera".

I think Star is quite a bit older here, compared to how she looked at the time of Right-Bang's departure. But the army of ducks is likely to have been conscripted gradually over years, so she has probably already had several before, just not that many.
