The "Age of Stars" is the ending you unlock by completing Ranni's questline, which you complete by pretty much literally marrying her by putting a ring on her finger and she gives you the Dark Moon Greatsword - Description for the sword: "A Moon Greatsword, bestowed by a Carian queen upon her spouse to honor long-standing tradition."
