I think I can see the inspiration for this, as I actually edited a blowjob picture to look like this a few years ago. A relatively common way to draw blowjobs is to draw it from a slightly elevated angle, so you see the face from above, and the lips are stretched out at the bottom of the face. It's basically shortcut that sometimes makes it looks like the girl doesn't have a mouth and is inserting the penis into her chin. Guessing this is meant to make fun of that, as it often looks weird.
