at the time of that comment, it was in a "Hilarious in hindsight" pool (not the case now)

and what I meant was the remark about Kisume's vicious habits wasn't made in an authoritative word of god fashion but that similar to a fan's opinion. Additionally, he remarked "I don't know what she does normally"

That was about Eirin, not Keine. Much more likely to be serious, if it weren't for the fact that Eirin is earth-born and so can't be that old, and that his tone was like "that'd be interesting right?"

That aside, regardless of ZUN's tone, it nevertheless seems true that Kisume indeed is a man-eater. Evidence comes from her previous appearances: her in-game titles were "terrifying well youkai" and "autumn man-eater". Plus, Aya remarks about her in Double Spoiler that she's a ferocious youkai that lurks in darkness targeting humans. So, the head-collecting bit might have been exaggerated, but she's a man-eater nonetheless. And you know how often Rumia is drawn with gruesome images.
