I don't consider this guro. Nipple piercings are very common in a lot of fetishes. Some, yes, even to this degree. (except these people get so many hooks in their body it puts Hellraiser to shame) I find this a very acceptable and sexy image... once you get rid of the clitoris piercings... ouch.
