There was the problem of living in a city without a functional sewer system. You can wash your hands all you want, if there is no clean water.

You've got it the wrong way around, sewer systems are for disposing of dirty water. You'd get your clean water from the well.
And yeah, in the cities people would throw out shit in the streets, but relatively few people lived in those cities.

Same problem as every city regardless of time period. Which is why each developed their own complex sewage and water transport system.
