The two, fighting so much, that their original hatred, rivalry, differences no longer mattered. They fell in love with the fight, knowing that it's something only the two of them could forever experience and enjoy, exclusively. They fight, shoot, burn, blast, and kill one another, but no longer out of hate. They both enjoy the challenge of fighting one another, testing the other, and being able to always put it your all. And when the day is over, both can shake it off, and enjoy a peaceful moment with one another, relaxing off of a mere daily routine workout from them, and reflect with one another about eachother's methods.
...I would actually love to see a manga like this. An immensely heated and passionate battle, stretching throughout the day, only to end in a stalemate, with both of them casually recovering from their wounds, relaxing, and talking as two would durring a coffee break.

The Hourai elixir doesn't just mean immortality; it means the perfect epitome of eternal youth. Therefore, any and all health problems they get would be only temporary. The Hourai elixir eliminates any illnesses, any blemishes or wounds, and perfects their bodies anew as a part of their immortality.
Ergo, they wouldn't even have to worry about rotted teeth or lung cancer. Really, they have no reason NOT to smoke because of all this, unless they really just don't like the taste.

That's actually what happened in canon. It's only a sport for them now. Mokou even admitted to herself that it's her fault that they are in this situation now and is generally pretty dependent on Kaguya's existence. Some dere mixed itself between all that tsun over the years.
