That got retconned, now all those failures were part of his master plan to destroy the imperium, with the 13th crusade being bumped from a modest success to the over-running of part of cadia that is still ongoing.

Not that it matters since he got a series of space-fortresses that blow up stars and failed blowing up cadia with them.
