It's more less confirmed that Miko used to be a guy and turned herself into a woman. Sanae shows a yen bill that has Shotoko's face on it, meaning that at the very least he is remembered as a man. The way in which Miko became a hermit let her change her form to whatever she desired, and according to ZUN she changed to fit the times.
