Proper insulation, effective central heating systems, and houses not built drafty as hell is a fairly effective countermeasure as most of the rest of the industrialized world has figured out. You can add a fireplace if you want but I've never been much of a fan aside from occasionally warming feet after spending time in snow boots.

To be fair, I doubt the kotatsu will catch on in warm places like the tropics and the African continent.
