Living in Gensokyo must really desensitize you to weirdness. I mean, her immortal friend's face is splitting into a gaping maw and all Keine can summon up in response is "Ah."

It's unlikely because it wouldn't last. The Parasyte's only inhabit where they end up in (typically the head, though some get stuck in the right hand, the jaw, etc).
While they can do some pretty deadly and outright horrifying resculpting techniques to defend themselves, if the body dies, so do they, unless they're able to jump into another body (hence eating someone head, severing an arm, etc.)
This parasyte is completely screwed because Keine can either rewrite history so it died without a host, or she can simply cut off or destroy Mokou's head and Mokou would regenerate back to normal. And Keine wouldn't even have to feel guilty about doing either one.

More to the point, the Parasyte shouldn't even be alive. If it gets the target's head, the target is dead, but Mokou's immortality should prevent that. Meaning she'd grow a new head and shed the Parasyte-infected head, leaving it without a host and one step away from being roasted. The same applies for most youkai as well.
Gensokyo would be a horrible place for the Parasytes to live. Even normal humans can tell when someone has been replaced by a Parasyte under certain conditions (like that one old woman who knew the Parasyte wasn't her daughter simply because of a combination of motherly instinct and that the Parasyte couldn' properly fake her daughter's reactions), nevermind the more formidable humans of Gensokyo.
